For the first time in history, Medicare has negotiated prices directly with drug manufacturers. These aren't small, 5% coupons. We’re talking about Maximum Fair Prices (MFPs) that represent discounts ranging from 38% to 79% off the 2023 list prices.

These prices officially took effect on January 1, 2026. If you are taking any of the following 10 medications, your out-of-pocket costs just plummeted:

  • Januvia (Diabetes): Now $113 (79% off)
  • Fiasp/NovoLog (Diabetes): Now $119 (76% off)
  • Farxiga (Diabetes/Heart Failure): Now $178.50 (68% off)
  • Enbrel (Arthritis): Now $2,355 (67% off)
  • Jardiance (Diabetes/Heart Failure): Now $197 (66% off)
  • Stelara (Psoriasis/Crohn’s): Now $4,695 (66% off)
  • Xarelto (Blood Thinner): Now $197 (62% off)
  • Eliquis (Blood Thinner): Now $231 (56% off)
  • Entresto (Heart Failure): Now $295 (53% off)
  • Imbruvica (Blood Cancer): Now $9,319 (38% off)

Note: These prices are for a 30-day supply. While $9,000 for Imbruvica still sounds high, remember that this is the price the plan pays: and it brings you to your out-of-pocket limit much faster (more on that in a second).

The $2,100 Safety Net You’ve Been Waiting For

Here is the "secret" that many people miss: while the negotiated prices are great, the real hero of 2026 is the new annual out-of-pocket cap.

In years past, if you had high-cost drugs, you could end up spending $5,000, $8,000, or even more in the "Donut Hole." In 2026, the maximum you will pay for covered Part D drugs is $2,100 for the entire year.

Once you hit that $2,100 mark: which includes your deductible and copays: your cost-sharing for the rest of the year drops to $0.

This change alone is expected to save Medicare beneficiaries an estimated $1.5 billion this year. For the roughly 9 million seniors nationwide who use these high-cost drugs, costs could be cut by more than half.

The "Smoothing" Rule: Spreading the Cost

I often hear from clients who are worried about hitting that $2,100 cap all in January or February. If you take an expensive medication like Eliquis or Xarelto, your first fill of the year could be quite expensive as you work through your deductible.

That’s where the Medicare Prescription Payment Plan (often called the "smoothing" rule) comes in. This is a voluntary program that lets you spread those out-of-pocket costs over the course of the entire year. Instead of paying a large sum at the pharmacy in January, you pay a monthly bill to your insurance company.

Is This Too Good to Be True? (The Fine Print)

As an independent broker, my job is to give you the straight talk. While these savings are massive, there are a few things you need to know:

  1. Part D vs. Medicare Advantage: These negotiated prices apply to all Medicare Part D plans. If you have a Medicare Advantage plan that includes drug coverage (MA-PD), these prices apply there too. However, they do not apply to drugs covered under Part B (like those administered in a doctor's office).
  2. Formularies Still Matter: Just because a drug has a negotiated price doesn't mean every single plan will cover it the same way. You still need to make sure your specific medication is on your plan's "formulary" (list of covered drugs).

Looking Ahead: Ozempic, Wegovy, and Beyond

The 10 drugs listed above are just the beginning. The "secret" for the next few years is that this list is going to grow significantly.

  • 2027: Medicare will negotiate prices for 15 more drugs, including popular GLP-1 medications like Ozempic and Wegovy.
  • 2028: Another 15 drugs will be added to the negotiation list.

This means that even if your specific medication isn't on the list today, there is a very high chance it will be soon.

Nathan’s Practical Tips for 2026

How do you make sure you’re actually getting these savings? Don't just assume your plan is doing it for you. Here is what I recommend:

  • Check your Explanation of Benefits (EOB): Look at your monthly statements. Are you seeing the lower prices for the 10 drugs mentioned? Are you getting closer to that $2,100 cap?
  • Compare Costs Every Year: The Medicare Annual Enrollment Period (AEP) runs from October 15 to December 7. This is the one time of year you can switch plans without a "qualifying event." Even if you like your current plan, the changes in 2026 are so significant that you must run a new comparison to see if a different plan would save you more under the new rules.
